A written plan before the appointment. Every patient gets a specific plan for implant number and placement before the day of surgery, not decided in the moment.
A backup plan for bone loss. Chicago patients often arrive years after losing teeth, when bone has already changed shape. The standard requires a documented alternative plan for these cases, not a hope that the original plan still fits.
A long-term follow-up schedule. Same-day refers to the day teeth go in, not the end of care. Dr. Atcha’s standard sets a follow-up schedule that continues for years after the procedure, tracking how the implants and the bite hold up.
“I built this because I got tired of hearing about same-day implants as if the day was the whole story,” Dr. Atcha said. “The day is one part. The standard covers the rest.”
Why he is publishing it now
Dr. Atcha has worked with complex dental patients in the Chicago area since 1996. Over that time, he has watched same-day implant treatment become more common, and more inconsistently defined.
“Patients hear ‘same-day’ and assume it means the same thing everywhere,” he said. “It doesn’t. I wanted a standard I could put in writing, so patients know exactly what they’re getting when they come to me.”
He points to the imaging step as the one most often skipped. “You cannot plan a same-day case off a guess. The scan tells you what’s actually there. Skip that step and you’re improvising in someone’s mouth.”
What this means for patients

He recommends patients ask directly whether a full scan happens before surgery, whether they receive a written plan in advance, and what the follow-up schedule looks like after the procedure.
“None of this is complicated,” Dr. Atcha said. “It’s just discipline. Do the imaging. Write the plan. Have a backup. Follow up for years, not months.”
He describes the standard as a floor, not a ceiling. “Every patient is different. But the floor should never move. That’s what makes same-day implants safe to offer at all.”
Looking ahead
Dr. Atcha says the four-part standard will now apply to every same-day implant case his practice takes on, regardless of complexity. He plans to keep reviewing it as imaging technology and implant materials change, but says the core requirements, imaging, planning, a backup plan, and long-term follow-up, will not shift.
“Thirty years in, I’ve learned that patients don’t remember the day of surgery nearly as much as they remember whether the plan held up five years later,” he said. “That’s the standard I’m holding myself to.”
